When you check the box that says "I would like to be considered for MD-only if I am not for the mstp," how much of a disadvantage are you at for admission?

I don't know what you mean.

1. Disadvantaged for MSTP admission? Not at all (at least not here). Programs realize MSTP is competitive and it it's reasonable to consider Medical School followed by research fellowship as an alternative career pathway.

2. Disadvantaged for MD admission? It would depend on when you application gets forwarded for review, which is highly school dependent. Yes, conceivably if your application isn't forwarded until March you might be at a disadvantage vs not applying vs MSTP. But this is a back-up option, right?

I think you are focusing a little too much on tactics with this question. Apply early (sometime over the summer, doesn't have to be the first week applications are out) to the programs you would want to go to and if you have the grades, MCAT, and research experience things will work out (and if not, there isn't much you can do about it other than improving these three).
