If a fellowship trained child psychiatrist is just hired by an adult psychiatrist, is it "good business" for the practice (two docs) to have the child psych start seeing adults until more children enter the practice. Would it not be financially worthwhile to see adults and children for medication and therapy?

First consider 2 factors:

1.Child psychiatry generally takes more time.
2. 3rd party payers do not value your time.


When does it make financial sense for a child psychiatrist to see children and not adults?

3 scenarios:

1. You negotiate a substantially higher rate with the insurance company for the child psychiatrist to see children.

2. You are out-of-network and set the child rates higher.

3. You are an employee and your employer provides a slightly higher salary for having the child fellowship.

In the case of a start up practice it makes sense to split the caseload until the child referrals match or exceed the adult ones.

I think splitting the load is a good idea. It could take awhile for the child referrals to start coming in, even if you are already paneled with a few of the insurance companies.
