I ran across this in kaplan notes and wanted to get other's imput :
"erythromycin has been shown to decrease neonatal Dz in infected mothers" but since erythro is not recommended in pregnancy ( also kaplan) can we just use azythromycin? since it is a co DOC for the infection in general?

The CDC reccomends erythromycin as first line therapy in pregnant women. I don't know where you read the pregnancy risk with erythromycin. Azthromycin is an acceptable second line therapy in pregnant women, I'm not certain why erythromycin is preferred, but I imagine that it has to do with it being better studied in the treatment of chlamydia.

I think you might have mixed up the pregnancy thing. The DOC for chylamydia is Doxycycline or azithromycin. HOWEVER, as you know you wouldnt use doxycycline (since its a tetracycline) on pregnant women (cuz of the fetal malnormalities--cartilage, teeth problems etc).

Having said that, Erythromycin is the DOC to prevent Inclusion Conjunctivities in newborns and is prophylactically put into the kids eyes as they come out of the birth canal. In addition, to treat infant chlamydial pneumonia you would treat w/ oral eythromycin. The reason you use ERYTHRO is cuz chlamydia doesnt have cell walls and so you are inhibiting protein synthesis. In terms of azithromycin I suppose you could use that but its only SECOND line.

thats essentially what I thought but like i said I got confused because kaplan pharm stated that erythro was "not recommended" during pregnancy...it wasnt talking about any specific dz, just drug info, so I wanted to verify with someone.
