In regards to the actual procedure itself, does anyone have problems consistently not freeing the mucous membrane from the glans and that becoming an issue when you retract the foreskin (mostly with the Mogen, but this initial step is present with the Gomco as well)? If so, what helped you solve that problem?

I've never used Mogen so cant comment on that, but I do know the Gomco procedure so I know what you are talking about.

I think the problem you are having is that you are not using the straight clamps enough to sweep around underneath the foreskin before you make your dorsal slit. When I first started doing circs, I would only do 1 quick sweep 180 degrees over the dorsal part of the glans. When I started doing multiple sweeps and opening up the clamp slightly inside the foreskin to stretch it out a bit more, then I found that the separation is much better when you make the dorsal slit. It greatly shortens the time you have to spend retracting the foreskin after you complete the dorsal slit.

The other hard part of circs to me when starting out is that whenever I observed someone else, I could never see exactly what their fingers were doing when they were retracting the foreskin. As a result when I started doing them on my own, retractions took way longer than necessary and I had a lot of unnecessary blood loss. Now that I have the technique down a little more, I went from having probably 0.5 ml of blood loss to maybe 3-4 small drops.
