In a case involving a group of children aged 4-6, the following clinical signs developed a few hours (6-24hr) after consuming the play dish of these beans. The children vomited and developed fever and drowsiness. Some became unconscious and died and one child was admitted to hospital, and clinical analyses showed the following: the skin was pale, non-icteric (not jaundiced), blood pressure was low 88/33 mm and pulse was 56. The liver was just palpable, the pupils were dilated with low response to light. Serum glucose level was very low, hemoglobin was normal, total leukocyte count was high with 66 % PMN. Serum ALT and AST were elevated to almost 500 times normal, CPK and LDH were elevated as well as elevation of serum creatinine and creatinine kinase levels. The child was hypokalemic and BUN level was elevated. Urine analysis showed proteinuria but no hematuria. In spite of emergency treatments of I.V. infusion with K+, glucose and Vit K and bowel washes, the child went into coma and died some hours later of respiratory failure and circulatory collapse. These clinical signs described are very similar to Reye syndrome.
