Different schools have different ways of ranking students. Some schools determine class rank after you have finished your pre-clinical years. Some include pre-clinical grades plus USMLE/COMLEX I. Some include clerkship grades in addition to pre-clinical grades.
You may want to ask this question on your interviews. If you are already a medical student, do the best you can in all of your courses both pre-clinical and clinical.
