On of the activities I have listed is a fine arts competition that I’ve been involved in for 8 years (even before high school). I started out as a participant and once I enter college I became a coach.

I had a friend who matriculated at the med school I want to attend and he did the same program; he told me to include the years I spent as a participant as well as a coach because it shows commitment and that’s how he did his app as well.

My question is whether I should classify this as an extracurricular, non-clinical volunteer work (coaching), or a hobby since I loved it but it was never school-related.

You would need to pick one of the available tags that encompasses both roles (participant and coach) if you want to include the full timespan. When you were involved in the competition during HS, you were not a volunteer. When you became a coach, you took on a Teaching/mentoring role. So neither of those can be used. That leaves Extracurricular, Hobbies, or Artistic Endeavor. I'd pick the last.
