Can anyone give me an idea for the following case? (This is my hometask for gynaecology)

Ms.S, 19 years old, accounting student. She presented with 5 years history of irregular periods and worsening facial hair.

Her menarche occured at age of 13 years old and her cycle always been irregular, approximately 4-5 cycles per year.
She not using any form of contraception

Family history:

Her mother was operated 'hysterectomy' at age of 41 because heavy periods.

Clinical examination: Body mass index 35
Blood pressure 110/70
There're facial hirsutism and acne
Pelvic and abdominal examination were normal.


What's the initial diagnosis? What is the plan of investigation.
