What is the significance of having an elevated Urine Creatinine but a normal serum creatinine?

A largely elevated urine creatinine compared to serum creatinine can indicate the possibility of prerenal acute kidney injury where certain substances (e.g. creatinine) may be concentrated.

The great thing about creatinine is that its excretion into the urine is a constant rate proportional to the individual's lean body mass. Thus, one would secrete the same amount of creatinine into the urine per day regardless of whether that person drank 2 L of water in a day or just a cup. But the one who was less hydrated would make less urine and thus have more concentrated urine.

Thus, if the urinary creatinine is very elevated, that signifies a lack of adequate blood flow to the kidney whether that be from a myriad of causes including dehydration, hypovolemia, heart failure, etc.
