Clinical notes in various settings

I was wondering how Y'all document in various settings as Psychiatrists or as Residents i.e. Nursing Homes, IOPs, Partials, Emergency, Consult, IP

how much time do you guys spend on this and how does the structure of the note differ in your practice.
