What is a good resource for conducting the above reviews, that is non-acute care or low acute care rounding and interventions? Additionally, what common things in the chart do pharmacists look for? Have a nice day.

Drug utilization review: dosing, interactions, sub or Supra therapeutic doses, meds that require discontinuation due to (inappropriate duration of therapy, IV. To PO). The lust continues

Yes the lust certainly continues..

Don’t forget - gradual dose reductions, psychotropic indications, prn psychotropics, hold parameters, Levo dosing, drug monitoring via routine labs.

Don’t use a resource unless you need it such as Lexicomp. After reviewing about 5000 charts you should master it. Also you should not exceed an average of 6-7 minutes per chart
