Clonidine patch and tablet?

Have you seen a patient on both clonidine patches and tablets? Is there a benefit for a patient having part of their dose oral and part patch?

Were these new scripts? What about compliance? Missing a couple doses of clonidine could potentially lead to hypertensive crisis or so we were told in pharmacology. Plus the patch is less sedating... What doses are they on?

Is the patient going to be on both dosage forms long term? Or is he or she switching from oral to transdermal?

No they have been on both for like 3 months now. Nearly maxed out. 1.8mg total

is the po scheduled or prn? it could be for spikes in BP not controlled by the patch.

Had a patient placed on Clonidine patch starting at 1900. Before I faxed the order her only prn med for BP was Clonidine 0.2 Q6 SBP > 180.

Anyways I faxed the patch and immediately pharmacy added it and removed my PRN medication. The order did not say chance clonidine, or anything to indicate the doctor wanted it removed. So I gave it later anyways (had to override).

Was this possibly detrimental? I didn't think so.

Just a coincidence you guys are discussing this and had a similiar dilemma at work the other night, not trying to hijack.

the question that I would ask is what is the patient's BP? It is under control? Does the patient have any side effects?

Update: I called to verify and the MD says they are indeed both on it.

No I never even got to speak to the MD. I just got a message from the secretary that the patient would have a follow-up appt the next day so the issue would be looked into then. Then i got a message via fax that yes. patient is on both.
