I just completed my sophomore year in undergrad, so I am starting to look deeper into actually applying to med school. From what I've researched, I should send the application the fall of SENIOR year (assuming no gap year).

Does this mean that med schools will receive my transcript for freshman, sophomore, junior year only? In other words, would the GPA from my first three years of undergrad be the GPA that will be used by medical schools for my application?

You apply the year before you plan to start medical school. The application opens in early May, and can be submitted in early June. For example, I submitted my application in June of 2012, and am starting medical school in August of this year.

And to the second part: the schools are going to see the grades that you submit on your application. You can update the schools as you receive new grades.
