If instead of just one first-author there are two first-authors, how is this clarified in the title?

In other words, if two authors both did the major amount of work, at equal quantities, then how is this reflected in the author list?

There is usually a mark (* or some such) by each name and a disclaimer at the bottom or wherever the institutions of each person are that says, "these authors contributed equally."

Incidentally, I recently saw THIRD AND FOURTH authors on a Cell (I think) paper that were listed as equal contributors. A sandwhich is a sandwhich, I think.

Wow, third and fourth author sharing, must be some real politics going on there.

I'd be happy to just get my name on a Cell paper!

Agreed. I'd take the Cell paper in a heartbeat - even as the penultimate author. But can someone tell me when being 3d or 4th would make a difference??? Maybe I'm imaginationally impaired (yes, I made that up, but you know what I mean), but when would that matter?
