Just guessing, but one grain is 65mg and colchicine predates the FDA, so it’s probably actually 1/100 grain or something. Conversely, the Europeans used a reasonable metric amount.
I do believe the combo drug Colbenemid does use 0.5mg colchicine + 0.5g probenecid...interestingly the MAX dose is 4 tablets/day (>1.8mg colchicine). Might find minutia/details in the LoDoCo trial (2012?) re-evaluating the safest effective dose vs the historical dose used for colchicine

Dosage, as well as overdosage is calculated based on the average body weight of the patient.
Milder adverse reactions began from 0,5 mg/kg.
There was 100% mortality among patients who ingested more than 0.8mg/kg. (link)
I suppose that EU & US patient average body weight is different.
Do you realize who much colchicne you have to take to OD. If you take the imaginary 70 kg male, you would ahve to ingest 70 capsules to OD.
The difference in dosing is due to how you recon a grain. Same as Phenobarital and FeSo4. Phenobarb was made in grains. Depending on how you reckon a grain 60 mg, 64.8 mg or 65 mg.
The really fascinating thing is the study underway for Colchicine in Covid-19. The idea is it reduces inflammation without depressing the immune system. That coupled with the study just released in the BMJ from NYU that showed once admitted to the hospital, the biggest predictor of outcome aside from age was inflammatory markers.
So is it not based on a average body weight? Do u know exactly how Europe calculates grain?

The official and correct number is 64.8 mg/gr. Pharmacy as a comvention has used 60 and 65 with 60 most commonly used for Pb and 65 used for FeSo4. But it matters not it's still one grain. we used to have a 0.5 mg tablet by Abbott back in the day as well as the 0.6 mg.
