This is going to sound overscrupulous and paranoid but on my state license application it asks for suspensions/probations/withdrawal/resignations during "while at any professional school, or training program" - this just means medical school/grad school, correct? I was on academic probation in college, and wasn't sure if I should disclose it or not (i was pretty dumb, but worked through it, obviously got in to med school lol).

Undergrad wouldn't be a professional school or training program. I would say this doesn't apply. When in doubt, call the state board up and ask the question. You should be able to do that anonymously.

College doesn't count, and I doubt most PhD programs would qualify either. Professional school would include medical school, but also any of the other health-related professions- pharmacy, podiatry, etc and I guess also non-health-related professions- law school and maybe MBA
