I have never heard of anybody doing such a fellowship. Honestly not sure it's very beneficial. I would just do a regular PICU fellowship in this case.
No not common. In fact I don't know of anyone, but there might be someone out there who did this. I don't think it'll bring a significant advantage in taking care of children in the PICU. PICU fellowship trains you for most of the basic ID situations you will be dealing with, and the ID service loves to consult and round in the PICU when needed. Remember, many of the kids who are admitted to the PICU aren't primary infectious issues. If splitting your time between general peds ID and PICU is your thing, may be worth considering. But unlike peds cards/PICU, peds anesthesia/PICU, there's not a continuity piece between the two specialties.

How does he split his/her time? Motivation for that particular combination?

I know of one person who did this. To be honest, when I heard, I thought it was quite a colossal waste of time (and still think so).... I agree with Michigangirl - PICU training prepares you immensely for ICU infections, both common and rare. There is no added advantage in the ICU to being board-certified/trained in ID. So if the ICU is where you want to be permanently, then ID fellowship is a waste of time. If however, you have other interests outside the ICU, and want to do consults on interesting ID cases on the general pediatric ward, that's a separate issue - and you're more than welcome to. I also know someone who did combined GI and PICU. Know someone else who did Renal + PICU. So there are lots of combinations out there. Of course, Cardiology + PICU is the most common.
You may want to consider "cold emailing" someone you've identified as a PICU/ID attending to ask their advice. Faculty at Boston Children's and Columbia have done this.
