Combined pediatrics/medical genetics residency programs

I am going to apply for the combined pediatrics/genetics residency, but I am having difficulty finding a list of programs that offer this. I know new programs are being added every year. Does anyone have a list of programs?

Also, how do you go about applying for the combined program? ERAS does not list separate numbers for the combined programs.

I believe Cincinnati and Baylor have programs as well - classmate/friend matched Baylor this year.
