What happens if failure of COMLEX 2 happens TWICE?

your school will probably let you sit a third and last time after presenting it to the "committee" that is in charge of graduation requirements. Not sure if you'd be able to start a residency without technically graduation.
 
Know of a person this happened to. His program let him start late because you technically cannot graduate from DO school until you pass it. So, if you fail twice, hopefully not, but if you do, hopefully you'd pass on the third try and your program will let you start late.

It depends on the school. AZCOM only makes us take the test, not pass. (I think the class of 2008 will have to pass.)
