COMLEX PE hand washing

Just took my PE. I used hand sanitizer before the PE. But when I entered the room I shook hands with the patients before any type of sanitizing. I didn't get any comments or negative impressions from patients when doing this.
Is this ok? I've heard some people wash as the first thing when entering the room before any patient contact so it's made me a little nervous.
