COMLEX/USMLE out of IM

I'm not a fellowship director, but my impression is USMLE scores are less important than good references and the ability to obtain an unrestricted license. That having been said, if you are looking at highly competitive specialty (I'm looking at you GI), in a highly competitive region (e.g. California), you should consider doing both.
 
I think the general SDN consensus on this topic is, take Step, do well, match at a program with in-house fellowships (many of these types of programs will require a Step score), then go on from there.

Here is a source you can use to look for programs. I'm take the info with a grain of salt though. I know for sure one program on this website has mis-information w/ respect to average Step score for their residents, so that may very well be the case for others as well: FREIDA Residency Program Database | Medical Fellowship Database | AMA
