So, came in tonight to an empty ED at 7pm. Got 4 patients in the next hour, and two more just after 8pm. One pt was in the room, in the gown, but sitting in the chair - his friend was on the bed. I thought she was his girlfriend. Dude was 30, and had laid his bike down (a Triumph!), and was ground on the left side. Last week, while snowboarding on the mainland, he crashed and separated a shoulder, and sounds like he had a diastasis of the pubic symphysis. Well, he's all messed up. But I made a good connection with him and his(?) woman. It was so much so that she told me, with him in the room, that she just had surgery and had a question, and if I would look at it. I asked what was the surgery. She says, "breast augmentation". She pulls up the shirt and the bra, and there is near perfection - 9.5/10. There was some fibrosis at the scar line (not visible, but palpable). I said I didn't have to even buy her dinner. He says, "I bought her dinner last night and didn't get that far!" I had pulled the curtain (since the door was open), but she was standing with her back almost to him - both in the room (not the curtain between him and her).

Well, after discharge, the nurses tell me that they WEREN'T a couple - just friends. Oh my. Still, I didn't go further. Seeing her beautiful chest was more than enough, and not having the state come after me was reassuring.

But, WOW! I, as a rule, don't like implants, but, WOW!

How come you get all the "interesting cases"?

Right?! Two days ago an attending was showing me how to do an US on a 70 yo guy with a breast mass… after we finish the guy says to me: “Is it my turn now?” with the attending (male) right there…. 😱 😡 disturbing on so many levels...
