I haven't requested any official documentation. What I did do, and continue to do, is keep a file for each volunteer activity. It lists a brief description of the volunteer activity, dates and times i participated, name of organization and name and phone number of the contact person for that organization.

I'm only applying at one school this year (UMKC) and they only ask for contact person's name and phone number, no documentation.
