I will be applying to dental school this cycle and was just curious on how we submit our community service proof? Does it have to be on a letterhead of the company where I did community service and signed, or do I just simply write it down in my application and how many hours

No, you just report it yourself. It is semi-honor system.

Well, in most cases you won’t have to show proof. However, it is still possible that they might ask for proof. One of the applicants I know of was asked to provide proof of membership in an academic club on our campus. I do not remember which school it was.
