Complete contact info necessary on AMCAS?

Sorry about trivial questions like this. I don't remember the last names of my contacts from two jobs. Should I enter just the first name or leave the contact info out totally?

Yeah, if i were on an adcom, i'd question how involved you were if you don't know the last names.

I've had probably 11-12 jobs over the last 12 years, so I would have no way in hell of remembering the full contact info for all of them. I'd bet I'm not alone in this respect, and it probably gets worse the older you get.
 
I agree with the suggestion to contact the employer. I had two summer internships quite some time ago and I contacted the HR deptt of the place. They will always have - When you worked at that location (date from-to), in the very least and a contact name/# for verification. In most cases that's all they can give out to anyone who wants to verify. In othercases you can get job title, how often you worked, etc.

IMHO, make the effort and pick up the phone if you're going to list it.

If these jobs are going to be put down as a "significant" post secondary experience, "knowing that medical schools are more interested in quality than quantity," I'd think you should know the full name of your contact.
 
I couldn't remember the last name of my employer in college, but since it was affiliated with the school and work study, I left it blank.

I guess I should have tried to track down the name of my old supervisor...
