Hmm from reading this seems like

Mayo > Penn/Duke > Brown (just bc you said far from home & you seem worried about student body makeup) > Hofstra
 
Based on your pros/cons, Penn. While the cost is higher—which may end up being the driving factor—it seems like you do not like what Hofstra has to offer in regards to living and also the school/curriculum.

Additionally, you list graded clerkships as a con for Penn but Hofstra also has graded clerkships. You also list "solid ranking" as a pro for Hofstra but I'm not sure if that is really a pro when you look at the other schools on your list especially when Penn is on there. So it seems like you're going to have to decide between a much cheaper Hofstra and being close to your support system vs. Penn. Could you clear up the finances a bit more? How much in loans?

My apologies for the confusion. I think the price is really what's holding me back from Penn. Penn is entirely loans (106K per year x 4 years = 424 before interest).

Hofstra is roughly 60K per year in loans (240K over 4 years)

That's certainly an insane amount to take out in loans. I'm not expert on how exactly that would add up via interest (assuming a boatload) but I would ask, have you asked Penn to match the half-tuition scholarship? Even though Hofstra isn't exactly in the same tier as Penn, I have heard that Penn is pretty generous in matching aid so maybe it's worth a shot.

Penn is refusing to match any scholarships this year unfortunately (many other students have also been denied a match by Penn for schools in the same tier)
 
If you don’t know what you want to do and want ultimate flexibility it’s hard to pass up Penn. What specialty are you leaning towards or are you undecided?
IR, vascular surgery, or anesthesia are my main interests (but could always change)
 
Penn has decent number of traditional (no gap year) students
 
This is a tough choice but I would lean Penn
