Medical Dermatology Fellowship

Complex medical dermatology fellowship training is designed to permit the acquisition of knowledge and experience that will aid in the care of patients with complicated medical dermatologic illnesses. This includes, but is not limited to, the management of autoimmune connective tissue and blistering disorders, cutaneous vasculidities, lymphomas, severe dermatoses requiring toxic immunomodulatory therapies and leg ulcers, as well as the recognition of the dermatologic manifestations and implications of systemic diseases. This training will prepare the fellow for careers in clinical or academic practice. The training program will include: 1) Clinical participation in medical dermatology patient care and management, learning the evaluation and management of complex manifestations of cutaneous signs of internal disease, the use of immunosuppressive agents, appropriate use of medical consultants and the use of technology to deliver specialized care. 2) Formal didactic learning focusing on the review of relevant disease states in both classic dermatologic texts and in subspecialty publications as well are regular review of key dermatology and internal medicine journals including but not limited to: J Am Acad Dermatol, Brit J Dermatol, JAMA Dermatology, NEJM, Int J Dermatol, Arth & Rheum, J Dermatol Ther. Key components of the syllabus will mirror the medical dermatology components of the American Board of Dermatology guidelines for dermatologyic training (see below). 3) Investigational participation in retrospective reviews and prospective clinical trials under IRB approved protocols, along with experience in developing and publishing manuscripts in key dermatologic journals as listed in #2. The Medical Dermatology Fellowship is ideal for a motivated individual who wishes to gain advanced clinical experience in the management of complex medical dermatology patients. The fellowship will provide rigorous clinical training in a private practice setting specializing in the care of patients with cutaneous lymphoma, autoimmune skin diseases including vasculitis, connective tissue diseases, bullous diseases, psoriasis, atopic dermatitis, and wound care along with practical management strategies for systemic medications and treatments used in these conditions. An integral aspect of the position will be for the fellow to participate in ongoing industry-sponsored clinical trials. The fellowship duration can be 1-2 years and will provide training that is ideal for those interested in a future career in academic medical dermatology or any individual who wants to feel more confident in diagnosis and management of these challenging patients. Prerequisite training is successful completion of a dermatology residency program and a valid Michigan medical license. Locale- FIVENSON DERMATOLOGY is located in Ann Arbor, MI. Ann Arbor is a mid-sized Midwestern city and home to the University of Michigan. As a Big 10 community it offers a diversity of sports, arts, music, theater, etc.. There is also professional baseball, football and basketball nearby in the Detroit Metropolitan area. As a college town, Ann Arbor is a great place for young families and offers many excellent schools. Relationship to host institution- The Dermatology Residency Program at St. Joseph Mercy Health System is an osteopathic training program established in 2006. It accepts 4 residents and one dermopathology and one Mohs fellow each year. Fellow in the Medical Dermatology Fellowship will be expected to participate as junior faculty and help supervise resident clinics, assist with inpatient consultations, attend monthly Dermatology Grand Rounds, present cases in the annual Michigan Dermatological Society Meeting hosted at St. Joseph Mercy Hospital and present papers at the annual Medical Dermatology Meeting. Background on Medical Dermatology Fellowship:
Medical Dermatology is a sub-specialty of Dermatology that encompasses cutaneous manifestations of systemic disease. Much of this field intertwines with Rheumatology, Oncology, and non-healing wounds of multiple etiologies. Medical Dermatologists see some of the most difficult Dermatologic cases, yet these specialists are few in number. There is a growing need for more of these specialists, who are often receiving referrals from surrounding states. Medical Dermatology is not an ACGME approved fellowship, but the field is overseen by the Medical Dermatology Society.

The fellowship is ideal for motivated individuals who wish to gain advanced clinical experience in the management of complex Medical Dermatology patients. The program will provide rigorous clinical training in a private practice setting, specializing in the care of patients with cutaneous lymphoma, autoimmune skin diseases including vasculitis, connective tissue diseases, bullous diseases, psoriasis, severe atopic dermatitis, and intense wound care. Practical management strategies for systemic medications, including immune-modulatory therapies, will receive particular focus. An integral aspect of the program will be for the fellow to participate in ongoing clinical trials of state-of-the-art medications and other therapies. In addition, the fellow will be taught how to recognize Dermatologic manifestations of underlying severe systemic diseases. The fellowship duration will be 12 months, with some room for individualization, based on the trainee's personal interests and aspirations. The program will provide training that is ideal for those interested in a future career in either academic or community-based Medical Dermatology. Prerequisite training is successful completion of an Allopathic or Osteopathic Dermatology residency, and a valid Michigan medical license.

The fellowship will provide rigorous clinical training in both a private practice setting and within the St. Joseph Mercy Health System, Ann Arbor, MI and it's Dermatology Residency Program’s outpatient clinic and inpatient consultation services. Dr. Fivenson has over 25 years’ experience specializing in Medical Dermatology. The Medical Dermatology Fellow will work closely with Dr. Fivenson on these complex patients and gradually become an expert in the care of these most challenging of dermatologic patients.
