When do you admit concussions?

I had a 13 year old who flipped over his motorbike and was knocked out maybe a few seconds. CT Head negative. Normal neuro exam except kept asking the same questions over and over againevery few minutes. Mostly just post-traumatic amnesia.

Peds won't admit b/c we don't have Peds Neuro. Do you transfer this kid an hour away or watch him at home? His parents are very good and has good f/u.

Mainly is admit warranted when really there's nothing you can do but watch and wait.

Generally, if you either can't control their symptoms (nausea/vomiting), they're unsafe for discharge (elderly and extremely wobbly on their feet) or they don't have a place to go with a reliable caregiver. While the perseveration can be a bit freaky, it tends to be short lived and with reassurance, most family members are comfortable with discharge with meds for n/v.
