Confused about letter request form for AMCAS with LORs

I noticed there is a letter request form in the LOR section of the AMCAs application. Do we need to print and mail this form to each LOR writer in order for them to post LOR's online at AMCAS?

Do I have to send LOR writers this form even if they are posting my LOR online, or only if they are snail-mailing the LORs to AMCAS? Any clarification would be greatly appreciated!

I used Interfolio for all my LORs (which were then sent to AMCAS) and did NOT use the AMCAS form. I had no problems.
 
Those professors who are familiar with submitting letters online (usually science professors who do it all the time ) need only the number that amcas generAtes for the letter. Any that will be snail mailed will need to be Accompanied by the letter request form.
