😕 Probably a stupid question, but I have to ask!

I notice some programs have a three year program and others have a four year anesthesiology program. Is it that the four year programs have a linked internship year? So as a fourth year would you apply only to linked programs and then if you did not match 😱 apply to three year programs during your internship.

Hope someone can shed some light on this. Thanks😉

Anesthesia training is 3 yrs, referred to as Clinical Anesthsia year one through three. CA 1, CA 2 and CA 3. This follows the internship year. There are programs that match you to all four years, while others match the Clinical Anesthsia years only and you have a choice for where to go for your prelim training. In anycase, all CA 1 years are matched together, one year earlier. So if you did not match, into a CA 1 position a year in advance, you most likely will lose a year, since those spots would be spoken for by those who did match.

A few programs keep CA1 positions open and match during the PGY 1 year. Virginia Mason, for instance, reserves a single spot to fill in this way.

In the past it was easy to match into anesthesia from another residency and enter as a CA1. This, of course, will be very difficult now with programs filling so easily. However, some programs are attempting to increase their class sizes and so it is potentially possible to match later.

Finally, there has been a national movement to force all Anesthesiology residencies to offer four year training. Most places that currently have three year programs agree that this is going to happen in the next 5 years. (Incidentally, any place that has only a three year program most likely considers getting a transitional/PGY1 year up and running a huge logistical nightmare. Places that have four year programs either have thought seriously about what the best training might be, or just hand you over to various departments to work you as they please. Of course, there is no hard evidence that any particular PGY 1 year is most appropriate, but there are a great deal of opinions. Just look at the variability of transitional years. Some have 2 months of electives, while others have 6 months or more. UNC PGY 1 has no electives.)
