Who do you guys use as a contact for an award such as Dean's List? Is it even worth to add this to my application?

I put myself for all awards and hobbies. It never came up as an issue. Any academic awards, such as dean's list, should be included if you have the space.

No contact needed. List your university registrar if necessary. Definitely note this achievement somewhere on your application, even though this honor is typically based on your grades which are reported elsewhere.

But it asks for first and last name of contact person, and their position?

I grouped all my activities together (dean's list, scholarships, cum laude, other recognition, etc.) and listed my academic advisor. She knows me well and could vouch for all but maybe one of them.

Exactly what it sounds like. Like if was shadowing, name it shadowing. If it was volunteering with Big Brother, call is Volunteering with Big Brother. For Dean's List, just call it Collegiate Honors and Awards. That simple.
