I'm going to be applying to dental school this summer so I'm starting to make a list of my ECs. For those who have already applied, do we have to list contacts and their phone numbers and emails on the AADSAS?

OK great!

I heard schools require documentation of our shadowing. Will we have those forms on the AADSAS? I'm getting a LOR from one of the dentists I'm shadowing but not the others.

On dentalworks' post of the AADSAS template, it seems that they ask for supervisors of dentistry experience, for EC organizations, and for work employers.

There doesn't seem to be prompts for EC references and their contact information.

Could people who have applied please tell me if I need to list references and their contact information for extracurricular activities on the AADSAS or school secondaries? My friends who have applied to med school told me that the AMCAS gives explicit instructions to include contacts and their info for ECs.

I'm starting to collect supervisors' info for my ECs but some activities are from 3 years ago and it would be a huge hassle to dig up contacts.

oh you are right. They only ask for the names and # of hours etc.

For example: in dentistry/Shadow section, they will ask you these info:

Name Of Supervisor
Position Title
Type Of Dentistry Observed
Brief Description Of Activities
Total Number Of Hours
Position Type
Month Of Start Date
Year Of Start Date
Month Of End Date
Year Of End Date
 
On the extracurricular section I saw these as the prompts:

Name of Organization
Position Title/Brief Description
Average Weekly Hours
Total Number of Hours Over Span of Experience
Month of Start Date
Year of Start Date
Month of End Date
Year of End Date

There is no prompt for a reference unlike for the dental experience section where you list your supervisor. Do they ask for contacts for extracurriculars in secondaries? Am I suppose to have my LOR writers mention my activities (i.e. clubs not related to what I did with my reference) for verification?
 
There's very little verification necessary, and it's fairly unlikely you'll want a LOR from a non dentist or professor. There's no need for them to verify your extracurriculars either, as you'll want them to speak on the areas in which they know you (which would be class, research, or shadowing).

The only school I applied to that wanted contact info in their secondaries was UCLA. And UCLA's secondary is annoyingly specific about everything...
