Hello, MS4 on my ICU sub-I here. Just a quick question about something that an IM attending told me the other day. He said that if you are interested in a patient's hemoglobin, it is actually cheaper for the patient if you order a CBC w/o diff than to order an H&H. I tried to find the answer online but it's ridiculously difficult to find prices for lab tests. At this point it is mostly curiosity. Anyone know? Thanks!

Probably depends on the lab.

But in general, "panel" tests are cheaper than ordering each individual one separately. Like if you order Hgb, Hct, plt, wbc all separately that will exceed the cost of a CBC. But one individual component is almost certainly cheaper (like just a platelet count or just a hemoglobin). Where it breaks down is when you order multiple components. I would guess that H&H would still be cheaper than a straight CBC but it might not be. At our lab, for example, creatinine + potassium is the same price (I think) as the full chem7. And if you add one more test it's more expensive. So just order the whole thing unless you only want potassium or only creatinine. And it very much depends on the specific lab. The lab typically runs the full CBC with every test anyway because it's easier and just reports one if only one is ordered.

If you are just interested in the hemoglobin, I would just order the hemoglobin. That is most likely going to be cheaper than the CBC.
