Could some one give any insight?

Option1.
H1b (from off CAP) work 1 year prelim
and then after one year training - (perk, work experience! bette resume for next match)
I will have to have 1 year gap.
then reapply match getting h1b in another institution and repeating PGY1( categorical,)

vs Option2
Just have a gap year now and apply this upcoming 2020, get categorical
get H1b once till end of my residency.

which sounds more realistically better?
and one more question, I will need to ask laywer for sure but.. if anyone who knows
even if I have 1 year gap, can I transfer off Cap H1b? or I need to get new another H1b?

I think this is a very specific and technical question. You are better off asking a lawyer. Most IMGs are on J-1 and those who get H1B mostly don't take gap years during training. Good luck.

I think it depends - Do you have something lined up for gap year? And how likely are you to match next time, I mean how strong is your CV? Finally can you make it another year without getting paid?

You cannot transfer H1B after a gap. In fact for the gap year you will have to obtain another visa. You have 30 days to renew or find a new visa.
