So I picked a great time to ask but does anyone know of programs where taking on a couple is something they either do or are very open to doing? Preferably in the midwest.

I think most places will and do. It can complicate vacation schedules and is easier to accommodate in larger programs, but I don't see it as a big disadvantage. At least not much bigger than couples matching in general. Couples matching is always a disadvantage for someone, but it is a priority that is justified.
