course equivalency statement

Excerpt from UoP requirements:
"Limitation on Community College Work: Courses taken at a community
college will be acceptable if they are transferable as equivalent to
predental courses at a four-year college.
Students should submit a
copy of an advanced standing evaluation form provided by the four-year
college or a course equivalency statement from the community college."

For those who graduated from 4-year college without transfer from community college, but then took some pre-req courses at community college for dental school requirements, I was just wondering if anyone has ever submitted so-called 'course equivalency statement' to AADSAS or dental school? How do I get it?
 
I really don't think it matters. If you rock the DAT and have good grades they really will not care. If your application is littered with ???? about grades, DAT, extracurriculars, etc, would they look deeper into that information.
