Courses transferred to NEW institution

So if I took classes at let's so X University but then were transferred to Y University. Under which school should I put the classes that I took at X under?
Do I have to put the classes under both?

Lastly, if I received advanced placement credit for a class... X University says it's 4 credits but Y University says it's 3 credits after transfer... which one do I go by?


These may be a dumb question but I just wanted to make sure.. thanks in advance.
