Cover letter for LOR

I just contacted a physician that I've been shadowing for a few months and asked for a LOR. She is willing to write one for me, she just wants me to email her a cover letter and I wasn't sure what to include? I was going to include where to send the LORs and the dates of when I shadowed. Anything else?
