does anyone know how i can get a hold of a host request form to stay with a student host during my interview at creighton. i've tried numerous times to get it from the admissions office, but it never seems to show up. thanks.

This is from my email invitation:

Please remember, your host is a medical student and may need to study, so please come prepared to occupy your time. If you have any questions please contact the scheduler assigned to your interview date before10 PM CENTRAL TIME!!!! When calling, please keep in mind that the schedulers are students and will be in class during the day. IF NECESSARY, you may contact the Host Committee Chairman, Eva Carignan at (402)-553-5419, [email protected].
