Did anyone who wrote the patient note find that they ran out of space on the HPI section and had to cram stuff into it? I had to do this and literally had a few centimeters of space left before the physical exam section. As a result, I had crammed into whatever space was left in the HPI all of the allergies, PMH, fam hx, meds, soc hx, and ROS (I just wrote per HPI for ROS). While it was legible, it was just plain old sloppy. I mean I'm pretty confident that I put all the necessary components (OPQRST, etc) so all the content is there. It's just messy. Anyone do this and stiff end up ok in the end?
I feel like the cases were really straight forward when I took it and the rest of the exam I felt good about. Obviously no one is perfect so I missed a few small things here and there, but I'm more concerned about my note looking stupid.
