Are you supposed to bring your own reflex hammer,pen light tuning fork to the CS exam or are these provided? Thank you.

Bring a White Coat and a Stethoscope. They supply everything else.
Technically, they have a White Coat and Stethoscope for you to use, but a limited number, so do not rely on that.
You are allowed to bring into each room ONLY a pen they give you, the clipboard they give you, the white coat you brought and the stethoscope you brought. I suppose they allow you shirt and pants also...
