cuffless trach and PPV

what do you guys do when a patient comes to the OR from ICU with a uncuffed trach and theres no time/access to put ina cuffed trach and you have to give pos pressure ventilation???

Not sure I understand your question. Why no time? It literally takes 20 seconds and the access is there by definition. You could lose the access or stir up bleeding if it is a very fresh trach (less than 1 week) but these patients always have cuffed trachs in my experience. Just ask your circulator to get you a cuffed trach and swap them. Do it over a bougie or tube exchanger if it makes you feel better. I always switch to a cuffed trach before starting a case..
