So I've heard schools have the students stand at a podium and open an envelope and read it in front of everyone to share with them where they have matched... Or everyone opens the letter at the same time to celebrate simultaneously. Now, I was wondering what happens to those who don't match, especially if you have to read it out loud to everyone? Or does reading out loud/sharing with everyone not even happen??

The peeps who didn't match have aready scrambled before match day. They know where they are going to be before the match day folks find out. People find out if they have matched or not matched on the Monday before Match Day. On Tuesday, they are scrambling into open positions and most will already have a position by midday on Tuesday.

Having assisted plenty of people (American medical school) who didn't match on scramble day, all have had positions within 1 hour of the scramble opening. We set up phone banks and fax machines for our scramble folks who will generally number 2 to 3 out of the class anyway. We have never had a grad not find a position either scramble or match. There are plenty of us with connections that make sure out students have a position even if it means making some calls.

So what happens to that student who is dead set on a specialty, is qualified for it, doesn't match it, and isn't willing to settle for leftovers in another field during the scramble?

If they aren't willing to take something that's available in the scramble, then they simply dont have a residency. Take it or leave it.

Depends on the specialty. Some can do a pre-lim year and reapply and others do a research year or two and reapply.

A prelim year would be in either Internal Medicine or General Surgery. If they arent willing to take a scramble position in another field and they didnt want to go into medicine or surgery in the first place....
 
As the others have said, you find out if you matched anywhere a few days before you find out where you matched. If you did not match, you spend the interim days "scrambling" for a remaining spot.
