If I am applying to multiple specialties and I have done a few extracurriculars specific to a specialty, then should I mention that in my ERAS application or my personal statement? I don't want either specialty to look at my CV and think "well, do you want to do this or that?" Say for example, if I did a peds observership and then cardiology research for a few months. Should I mention them in my PS for each specialty or actually put them down on my CV.

Your CV in ERAS is made up of the activities, experiences, and publications you list on your application. You can certainly tailor your personal statements for both specialties (as long as you are VERY careful not to match the personal statement for Specialty A to a program in Specialty B), but the ERAS CV will be a constant.

However, when programs look at the activities on the CV, does that affect their decision to consider the candidate for an interview. Say I apply to peds, FM and IM and my CV seems to have more peds related activities/experiences, will that shun away IM and FM programs to consider me for interviews? Would it be better to mention these peds activities in my PS rather than the ERAS CV?

For those specialties, nobody will care. But you should really pick one specialty.
