Why dont we get Kussmaul's sign in cardiac tamponade? We get it in chronic constrictive pericarditis.
It's theoretically possible, but I would imagine that you would have hemodynamic collapse before it becomes evident.
 
Aren't their veins already distended with cardiac tamponade (I mean, JVD is part of the triad)? Hence making a paradoxical increase in JVP on inspiration difficult or impossible to see (i.e, in tamponade there is already distention regardless of the respiratory cycle they are in).
