DAT Address Change

I applied to take the DAT with my new address but my ID still has my old home address and I won't be getting a new ID with the new address before my test date. So I was thinking I should request my address to be changed but I'm not sure how. They say just fax a request but do they have a form somewhere on the site?? If it's not a form, what are we supposed to include in the request? Like name, birthday, eligibility number, old address, new address, signature?!?! and what else?!?

Also, is it a problem if I show up with an 'out of state' ID to the test center??

You should call the Prometric testing center to ask them about IDs on test day. They'll know what you're supposed to do and if the address makes a difference or if the photo ID with your name is the important part.
