DAT bootcamp hole punching question

Hi everyone!

I ran across this DAT bootcamp PAT question and am confused about how the paper is folded in the second fold. I've never seen it folded like this before-is it folded into the paper?

Yes....so the 2nd fold should have 4 layers.

2nd fold top portion has 1 layer and bottom has 3 layers. When unfolded the punches all come downwords
