DAT scheduling questions

Where do you actually go to take the DAT?
Does the place you take the test at charge you for their time?
How far in advance do I have to schedule the DAT, and what is it like (the process) of scheduling the DAT?
 
You apply to take the test and then pay the fee for it ($415). This is the only time you pay anything, unless you reschedule your test date (cost of rescheduling varies depending on when it is done). About a week after applying you receive a DENTPIN and then you can schedule a date. You take the test at a Prometric testing center, they do not charge you any additional fee. Once you've applied to take the DAT you get a 6 month window of time during which you can pick any date that the testing center (which you can select based on location) has available.
