Step II Date and Time on comlex PE SOAP note

So, there is no place to put date and time on this SOAP note? Seems weird. Also, I plan to sign my name and title at the bottom outside of the boxes. I hope this is acceptable.

The notes have a code which are specific to you so you don't really need to date it. I just signed my name at the bottom.

They tell you on test day to NOT sign your note and not to date it. They don't want the graders biased.

Now that I think harder about it (since I took mine in April), I actually don't remember signing it. I think I walked out feeling weird since I didn't sign any of them even though I always did on rotations. I did make sure my handwriting was semi-legible
