I haven't, but I'm pretty sure the application process is the same for both! Once you're in, then you can opt to go the dual-degree route.

Can we decide to do DDS/PhD in our first year of dental school? Should we indicate on our application that we are interested in a dual degree this cycle if that is the case?
 
School-dependent, however it is generally difficult to enter as a straight DDS and then go to DDS/PhD.

Applications for DDS-or-DMD/PhD are generally rolling admission processes. The earlier you apply, the greater likelihood of acceptance. If you know you want to do a dual training program, it's best to identify that now, as the NIH/NIDCR funding mechanisms favor those who start out as dual students.
